Welcome to the website of Venn Primary Care Network (PCN).

Our family of local GP practices are working together to transform health services in the Hull area. Whilst aiming to improve all aspects of health provision, we have identified some clear local priorities that will receive our special attention. PCNs were formed to provide support to groups of GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in a local area. We build on existing primary care services  to enable greater provision of proactive personalised, coordinated and more integrated health and social care for people living close to home. 

Venn PCN Services

Menopause 

To arrange an appointment please contact your own GP Practice requesting a referral to the Menopause Clinic. An appointment will be arranged for a blood test. Once your results are available the GP will review these. You will then receive a text message/phone call to arrange your initial appointment. For more information, click below.

Joint Injections 

To arrange an appointment please contact your GP Practice requesting a referral to the Joint Injection Clinic. An appointment will be arranged for you to see your GP. GP will assess if appropriate for referral, a referral to the Musculoskeletal Clinic may be offered initially.

LARC 

Long-acting reversible contraception (LARC) is the term used for birth control that helps prevent pregnancy after just one treatment. This service is open to patients aged 16+. To arrange an appointment please contact your GP Practice. For further information, click below:

Blood Pressure

A blood pressure test checks if your blood pressure is healthy, or if it's high or low. Blood pressure is the term used to describe the strength with which your blood pushes on the sides of your arteries as it's pumped around your body.You can check your blood pressure in your GP reception area and hand the reading print-out to staff.

Musculoskeletal

There is a twin-track process for musculoskeletal referrals. For certain conditions, patients can self-refer via a triage portal. Alternatively, they can be assessed by their GP who will discuss the most appropriate response for the presenting symptoms.

FeNO

FeNO testing can be arranged by your own GP practice. Fractional exhaled nitric oxide (FeNO) is a non-invasive biomarker used in the diagnosis and management of airway inflammation, particularly in asthma.

Social Prescribing

Social Prescribing is a community-based service that enables GP practices to refer patients to a range of services that offer support for social, emotional or practical needs. Through a link worker, social prescribing connects patients to community groups and services. For more information, please click below:

BeWell

The Health & Lifestyle Practitioners are non-clinical practitioners located within your GP practice who can support patients with making positive lifestyle changes including improving diet and increasing physical activity levels. They  focus on the prevention of conditions such as Cardiovascular Disease and Type 2 Diabetes. Please contact your GP practice if you would like to be referred to the BeWell Service. Spirometry

Spirometry is a simple, non-invasive procedure and common diagnostic test used to assess lung function and detect various respiratory conditions, including asthma, chronic obstructive pulmonary disease (COPD), and pulmonary fibrosis. It does so by measuring the volume and flow of air that a person can inhale and exhale. During a spirometry test, a patient breathes into a device called a spirometer, which measures the volume and flow of air expelled from the lungs.

Extended Access

Extended Access is an extension of your GP practice and offers routine and same day appointments with a range of clinicians including GPs, Nurses and other Healthcare Professionals.  Our practices within the PCN will be able to provide enhanced access appointments between the hours of 6.30pm to 8pm Mondays to Fridays and between 9am and 5pm Saturdays and Sundays.

ABPI

Peripheral arterial disease (PAD) is a common condition where a build up of fatty deposits in the arteries restricts blood supply to the leg muscles. Many people with PAD have no symptoms but some develop a painful ache in their legs when they walk, which usually disappears after a few minutes' rest.  An ankle brachial pressure index (ABPI) test is a painless test which compares the blood flow in your ankle with the blood flow in your arm. Contact your Practice to arrange an ABPI test.

Online Services

All Venn GP Practices operate convenient ONLINE SERVICES which can be access via the practice's own website. Requests for administrative support, prescription requests and links to non-urgent medical advice portals can be accessed 24/7. Requests for clinical service by the medical staff at the surgery are available during operating hours via Systmconnect. Non-urgent medical advice - including dental support - can be obtained 24/7 through the NHS 111 service by dialing 111 or via 111 Online. For medical emergencies, dial 999

Home Visiting Services

Our Home Visiting Team consists of Advanced Care Nurse Practitioners. The team support all our practices within the PCN by conducting home visits for housebound patients who are too unwell to leave their home or patients within a care home. The team are supported by GPs within the practices.
