Mental Health Coordinator

Severe Mental Illness Reviews are important to provide in the community due to the susceptibility of people with mental health conditions to develop physical health problems. These include but are not exclusive to, diabetes, cardiovascular disease, hypertension and respiratory disease.  

Patients are registered onto the Mental Health QOF, which consists of those who are prescribed antipsychotics, and those with a diagnosis of a secondary mental health condition. Reviews are important to determine how symptoms are managed and to determine if intervention is required. Side-Effects can be apparent, and these are monitored within reviews.

HBA1C bloods and lipid bloods alongside ECG’s are important as a way of assessing physical health and if patients need additional ongoing care, or outside referrals. In addition, within review’s a patient’s BMI is obtained. Outreach into the community, is an integral role to provide emotional and practical support.

With each contact, assessing the care needs required with each patient. These reviews are documented and can be seen by GP’s creating an important timeline into the patients care.

Patient’s all have different experiences and mental health presentations. It is important to provide a voice within the community, and provide support and regular check-ups as a way of signposting if required, to create lasting change and stability.
